ABOUT THE ROLE
We are looking for aCare Partner, Registered Nursewho is a passionate health and aged care professional looking for a new opportunity. As Care Partner you will be an integral point of contact for workers and clients to provide support for the broader community team. Join us on a permanent full-time capacity at Right at Home Southern NSW, located in Wagga Wagga.
To be successful you must hold:
* Formal tertiary qualification eg. Bachelor of Nursing or equivalent
* Current AHPRA registration as a Registered Nurse (Mandatory)
* National criminal history/police check
* Valid First Aid and CPR certificate
* Valid Working With Children's Check Card
Experience:
* Knowledge of the aged care industry/issues or Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles and the Aged Care Quality Standards – Essential
* Previous experience in Aged Care or related discipline – Desirable
* Easily adaptable to new software platforms
As aCare Partneryou will be responsible for:
* Building strong and meaningful connections with clients and their family
* Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home
* Interacting and providing professional health and clinical care advice and direction to a team of home care workers
* Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction
* Ensuring all practices are consistent within policy and legislative frameworks
* Maintaining up to date client records and information
