Dambimangari Aboriginal Corporation (DAC) is currently seeking motivated Care Navigators to act as the first point of contact for members seeking support with health, education, housing, or wellbeing needs. Central to the role is building trusted and culturally respectful relationships with individuals and families, providing guidance as they navigate complex service systems, and working to remove barriers that prevent access to the support they deserve.
This is a Full Time Role based in Derby, WA.

The Role:
You'll draws on your expertise in mental health, alcohol and other drugs (AOD), and holistic care to build trusted, culturally respectful relationships with members and help them access the right services at the right time, while also fostering independence, resilience, and community aspirations through practical support and referrals.
Key responsibilities include:
* Holistic Care Navigation - Provide trauma-informed, client-centered support by developing, implementing, and optimising individualised care plans that address complex needs, including mental health and AOD challenges.
* Continuous Client Monitoring - Track progress, provide timely guidance, and adapt care plans to ensure they remain responsive to members' evolving needs.
* Multidisciplinary Collaboration - Strengthen communication and coordination between GPs, mental health professionals, AOD specialists, and local community healthcare providers to deliver integrated, culturally safe care.
* Healthcare Outcome Optimisation - Build strong partnerships and apply proactive strategies to improve health, wellbeing, and overall outcomes for members.
* Program Development - Contribute to the launch, delivery, and continuous improvement of the Care Navigator Program, enhancing care models across the Dambimangari network.
About you:
You're proactive, culturally respectful, and committed to client-centered care. You bring:
* 3+ years' experience supporting clients with complex needs, including mental health and AOD.
* Strong case management skills, including risk assessment and planning.
* Knowledge of mental health services and referral pathways, especially in the Kimberley region.
* Lived experience or strong connection with Aboriginal and/or Torres Strait Islander communities (highly desirable).
* Ability to work collaboratively in integrated care teams.
* Empathic, innovative, and professional approach to client and community support.
* Excellent organisational, communication, and problem-solving skills.
* Experience in program design, implementation, and evaluation.
* Skilled in motivational interviewing and client engagement.
* Commitment to culturally safe, multidisciplinary care.
* Proficiency in Microsoft Office and record-keeping systems.
Qualification & Requirements:
* Qualifications and/or experience in community support, nursing, allied health, disability, mental health, or AOD.
* Ability to obtain and maintain:
* A Working with Children Check (within last 6 months)
* A National Police Clearance (within last 6 months)
* Undertake pre-employment workplace medical including drug & alcohol screening
* Highly desirable:
* Certificate IV or higher in Community Services, Mental Health, or AOD
* Familiarity with NDIS, housing, justice, and education systems.
