About the Role:
Join our team as a Grade 2 Social Worker in a full time, permanent role with our Department of Endocrinology and Diabetes (DPED) and our Adolescent team at Monash Children's Hospital, based at Clayton.
The Grade 2 Social Worker will provide essential clinical care and manage adolescent caseloads. This role is pivotal in offering comprehensive support to children and families dealing with chronic health conditions, ensuring they receive psychosocial support, advocacy, and access to community resources. Key responsibilities will include:
Deliver clinical care and support to children and families managing medical diagnoses such as diabetes, endocrine disorders, and eating disorders.Provide psychosocial assessments, counselling, and discharge planning.Collaborate with multidisciplinary teams to deliver holistic care.Assist families in developing coping strategies and navigating the health system.Supervise clinical staff, Allied Health Assistants, and students.Participate in after-hours on-call services and monthly evening clinics.This is a unique opportunity to join the Allied Health team at Monash Children's Hospital. About Us
Social Workers at Monash Children's Hospital are part of the broader Monash Children's Allied Health Team, and together we provide specialised services to infant, paediatric and adolescent patients and their families. Social work services are provided in inpatient settings including Paediatric ICU, Children's Cancer Centre, Neonatal Intensive Care Unit; in ambulatory care services such as the Department of Paediatric Endocrinology and Diabetes and we work closely with state-wide services such as the Victorian Paediatric Rehabilitation Service, the Victorian Paediatric Palliative Care Program and the Victorian Forensic Paediatric Medical Service.
The DPED and Adolescent teams offers a dynamic and supportive work environment. This position encompasses both inpatient and outpatient responsibilities with a strong emphasis on supportive care and counselling as well as engaging and collaborating with families to achieve optimal health outcomes for our patients and their families. A holistic approach to care is prioritised and utilising a systems approach is essential. This role operates in a fast-paced setting where the contributions of social work are highly regarded.
About You:
To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:
Dynamic and flexible team memberDemonstrated psychosocial assessment, problem solving, communication and discharge planning skills.Demonstrated skills in supporting families, understand the implications of acute and chronic illness on patients and families and be able to provide support in both instances.A strong understanding of end-of-life issues in paediatrics and be able to provide grief and bereavement support.Knowledge of resources and understanding of the assessment and intervention frameworks used to guide practice as well be familiar with the Children Youth and Families Act.You will be provided with training and required to participate in Social Work on-call after hour's service across the Monash Hospital sitesWhat you will need
Minimum 2 years clinical social work experienceBachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)Eligible for ordinary membership of AASW ( in an acute health care setting and/or with children and families and/or with Child Protection is advantageous.In Return
