We are seeking a skilled professional to provide high-quality support to children, young people, and families, ensuring their safety and well-being.
This role is ideal for someone passionate about making a difference in the lives of others. As a Support Practitioner, you will play a vital role in facilitating intake, screening, and placement of children and young people with foster and kinship care families.
1. Facilitate case management planning and develop tailored care plans that address individual needs.
2. Deliver assessments, crisis support, and therapeutic interventions; escalate concerns as needed.
3. Collaborate with community partners to deliver evidence-based parenting programs and support families.
4. Recruit, assess, and support foster and kinship carers, including facilitating approval processes.
5. Provide flexible, in-home support services, including after-hours and on-call duties.
Additionally, you will have the opportunity to work respectfully with Aboriginal and Torres Strait Islander communities to ensure culturally safe and inclusive practices.
Key Responsibilities:
* Develop and evaluate care plans that meet the unique needs of each client.

* Develop and implement effective case management strategies to improve outcomes for clients.
