Clinical Case Manager (Support at Home / Community Aged Care)

We are seeking an experienced and motivated Clinical Case Manager to join our growing community aged care team. This role is ideal for a Support at Home (SAH) Case Manager, who thrives in a fast-paced environment, values teamwork, and takes pride in delivering high-quality, person-centered care.
You will play a pivotal role as the central point of coordination for clients, families, and multidisciplinary stakeholders, ensuring care is clinically appropriate, well-coordinated, and responsive to changing needs.
About the Role
As a Clinical Case Manager, you will:
* Serve as the primary point of contact for clients, families, healthcare providers, social services, and community partners
* Respond promptly to client and representative enquiries, addressing concerns with professionalism and empathy
* Provide emotional support and guidance to clients and families throughout their care journey
* Facilitate clear, effective communication with internal and external interdisciplinary teams to ensure seamless, holistic care
* Advocate for clients' rights, needs, and best interests within the aged care system
* Perform thorough clinical assessments and identify, manage, or escalation signs of clinical deterioration
* Identify, assess, and manage risks while maintaining the client's dignity and rights
* Conduct comprehensive assessments and complete regular care plan reviews
* Develop, review, and update individualised care plans that reflect each client's needs, preferences, and goals
* Monitor and review client budgets, proactively managing expenditure and preventing overspend
* Support clients to upgrade their package level when care needs change
* Work collaboratively with the admin and rostering teams to establish appropriate, skilled care teams
* Adhere to roster workflows, ensuring shifts are covered and services meet client satisfaction standards
* Maintain open communication with senior leadership team and other team members ensure alignment with organisational goals, policies, and procedures
* Contribute to continuous improvement initiatives across service delivery and care coordination
About You
You will bring:
* Experience as a Case Manager in community aged care / SAH (essential)
* Nursing qualification (required)
* Strong understanding of community aged care funding models, care planning, and budget management
* Ability to build rapport with potential and existing clients
* Excellent written and verbal communication skills
* A practical, solutions-focused mindset, you are a doer, not just a delegator
* Strong organisational skills and the ability to manage competing priorities
* A collaborative, team-oriented approach with confidence working across disciplines
* A genuine commitment to client dignity, choice, and quality outcomes
* Ability to speak Mandarin is highly desirable, but not required
