About the role

COMMUNITY PALLIATIVE CARE COORDINATOR – COMMUNITY NURSING

Fixed Term (16/02/2026 to 15/11/2026)

Up to 72 hours per fortnight

Colac Area Health is a values-based organisation providing a unique integrated

health service comprising of over 500 employees, providing Acute Care, Aged

Care, Community and Allied Health Services.

The role of the Community Palliative Care (CPC) Coordinator is to provide senior clinical leadership to the nursing team and coordinate quality, effective, efficient palliative care services in the community setting. The CPC Coordinator oversees the triage and intake of referrals, initial assessments, risk management and care planning.

What's required?

* Registered Nurse with at least 5 years post registration experience in a related field
* Post Graduate qualifications in Palliative Care and/or Oncology or working towards completion of qualifications

* Extensive Palliative care experience in a range of settings and provision of expert advice to care team members

* Demonstrated capacity to develop and implement quality improvement projects

* Demonstrated ability to provide expert consultancy and education to support high quality palliative care delivery

* Demonstrated capacity to manage resources strategically and with probity

* Demonstrated capacity to manage tasks, manage performance and continuously improve work practices to achieve results with time and budget constraints

* The ability to build rapport, negotiate and maintain effective relations with key stakeholders

* A consultative leadership style that promotes participation and involvement

* The ability to create a learning environment

* Demonstrated ability to work autonomously, including on-call availability

* Current AHPRA Registration

* Current Driver's Licence

* Current Working with Children Check
