About the Role
The primary purpose of the Primary Care Coordinator (Clinical Relationship Manager) is to ensure the development and maintenance of partnerships between Capecare residential aged care homes, GPs, and other community health care professionals.
* Develop strong collaborative relationships with GPs, allied health and pharmacists to create a partnership‐in‐care approach.
* Collaborate with GPs to facilitate efficient, timely, and responsive primary care for residents.
* Facilitate communication between GPs, Capecare residential staff, residents and families/carers.
* Facilitate communication between other areas of the healthcare sector and Capecare.
* Support knowledge and skill development among clinical and care staff.
* In consultation with the Quality & Clinical Governance Manager:
o Develop clinical protocols with GPs to guide clinical decision making, triage, assessment and clinical escalation.
o Maintain involvement and oversight of GPs providing treatment to residents.
o Develop and manage an Implementation Working Group in line with GPAIC guidelines.
About You
You are an experienced Registered Nurse, a strong relationship builder who can network and advocate for high‐quality care across the community.
Qualifications
* Bachelor of Nursing – AHPRA registration
* Postgraduate qualifications in dementia care, palliative care, primary care or mental health nursing (desirable)
* Minimum of 5 years' experience in aged care
* Experience in dementia and palliative care
* Demonstrated knowledge of the Age Care Act and Standards
