What You'll Do
In this role, you will work closely with Team Leaders, Physiotherapists, and care staff to deliver person-centred, goal-directed care plans for clients referred through the My Aged Care program.
Your responsibilities will include:
* Conducting home-based clinical assessments to evaluate clients' needs and functional abilities.
* Attending a community centre where you will work with and supervise allied health staff.
* Developing and reviewing care plans that focus on reablement and enhancing clients' independence.
* Collaborating with clients and their families to set achievable goals and ensure their needs are met.
* Providing assessments for home modifications and assistive equipment to improve clients' quality of life.
* Maintaining accurate clinical documentation in line with organisational standards.
What You'll Bring
To thrive in this role, you should have:
* A Bachelor's degree inOccupational Therapy, or an equivalent qualification.
* Registration with AHPRA.
* Experience working with older adults, including those from diverse backgrounds and those living with dementia.
* Strong communication skills and the ability to engage clients holistically.
* Excellent organisational skills to manage your own caseload effectively.
* Availability for full days of work (Monday to Friday).
* An understanding of STRC, Post-Acute Care, and Transitional Care.
* Knowledge of CHSP and NDIS funding and referral processes.
