Primary & Secondary Care Interface Facilitator - Ipswich


Job Description:

We are seeking a dedicated individual to join our team as a primary and secondary care interface facilitator based in Ipswich. This role will involve leading and managing liaison activities between primary and secondary care providers to ensure seamless integrated service delivery.

The ideal candidate will have excellent communication skills and be able to build and maintain effective partnerships with clinicians, service managers, and external stakeholders.

This role will primarily be based at our Ipswich hospital site but will also involve travelling to other sites and GP practices when required.

Key Responsibilities:

* Acting as the key liaison between primary care providers, GP practices, community teams, and secondary care hospitals specialist services.
* Building and maintaining effective partnerships with clinicians, service managers, and external stakeholders.
* Leading service improvement initiatives, audits, and evaluations to ensure best practices are shared across care sectors.
* Analysing referral data, patient outcomes, and other metrics to evaluate service efficiency.
* Attending local interface meetings, Integrated Care Board (ICB) sessions, and multidisciplinary team (MDT) discussions.
* Developing and implementing strategies to support integrated care pathways.


Required Skills and Qualifications:

The successful candidate will possess excellent communication skills, be able to work effectively in a team environment, and have a strong understanding of healthcare systems and processes.

A degree in a relevant field or equivalent experience is desirable.
