The Youth Housing and Reintegration Service (YHARS) is a dynamic, youth-focused team working across South-West Brisbane to support young people aged 12–21 who are at risk of, or experiencing, homelessness.

Your Role as a Support Worker
As a Support Worker you will provide a high-quality service experience through initial assessment and triaging of needs, providing warm and successful referrals to services and working in collaboration with the team.
Utilising our WMQ Frameworks and Model of Care, you will ensure service delivery is person-centred, proactive, reliable, comprehensive, holistic and practical, delivered in a manner that is respectful of the dignity and privacy of individuals.
Key responsibilities of this position include:
* Conducting initial assessments and intake with individuals to determine eligibility, client needs and risks with the aim of providing a targeted intervention.
* Providing information, advice, supported referrals and support brief intervention case management.
* Assisting the team in the development and delivery of activities and programs.
* Maintaining and update program resources, materials and information.
* Assist the team to administer the program's After Care Brokerage component.
* Developing and maintain professional communication and relationships with young people and relevant community stakeholders.
What We're Looking For
* Completed Diploma or Certificate IV in Human Services, Youth Work or related qualification.
* Experience working with young people using trauma informed, person centred practice principles.
* High level communication and interpersonal skills including experience in engaging and working with diverse people with varied needs.
* An understanding and commitment to social justice principles, confidentiality, and privacy.
Essential Requirements
* Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
* Relevant probity checks required by legislation and WMQ Limited policy.
* Current Driver's Licence and willingness to travel for work.
