Join the Hawkesbury District Hospital team as a Part‐time Temporary Social Worker (Level 1/2) (up to 05/10/2026). This role is part of the Nepean Blue Mountains Local Health District (NBMLHD) and focuses on supporting patients and families through illness, hospital admission and transition within the health system.

In this role you will:
* Provide psychosocial assessments and detail discharge and care plans in a timely manner for in‐patient patients.
* Work collaboratively with the multi‐disciplinary team to ensure smooth transitions of care.
* Support patients and families in navigating the emotional and social impacts of illness, treatment and hospitalisation.
What You Will Bring To The Role
* Degree in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW)
* Understanding of the psychosocial impact of unexpected hospitalisation
* Experience working in an acute or medical/rehabilitation setting
* Ability to complete assessments and formulate care plans quickly for inpatient patients
* Experience working as part of a multi‐disciplinary team
* High‐level written and oral communication skills
