We are seeking a highly skilled Clinical Pharmacist to join our team in providing expert clinical pharmacy services. Job Description
* Provide clinical pharmacy services to Surgical Services, including Perioperative Services and Acute Pain Services, to resolve medication-related issues, undertake medication reconciliation, optimise medicines management, provide medicines information, medication education, and assist with discharge planning.
* Participate in appropriate ward rounds and planning meetings.
* Participate in pharmacist, intern, and student education programs.
* Initiate and participate in clinical pharmacy research projects and present at professional conferences, where appropriate.
