As the Transition Support Teacher you will develop strong networks and partnerships across agencies with a focus on providing necessary support and intervention for students and families from remote communities transitioning from primary school to secondary boarding school.As the Transition Support Teacher you will have responsibility for (but not limited to) the following: - Develop strong networks across agencies with a focus on provision of necessary support for students from remote communities. Support eligible young people who are in the community and who have exited early from boarding school to re-engage with learning or other pathways through a case management process ensuring students have access to support and interventions that enable them to complete year 12 or equivalent. Facilitate communication and/or mediation when required between students, parents/carers, school and residential staff and other relevant support agencies including ABSTUDY. Consult and collaborate with TSS Secondary Transition Team to support the parents/carers of current Year 7-12 boarding school students to connect with their child's boarding school and to engage with their child's secondary schooling. Advocate and take action as required in situations that impact on student enrolment, engagement and retention at school and in the residential facility.
