About the role
As the Primary Care (PC) intake worker you are an entry point for referrals to PC and connects consumers to the most appropriate services at Barwon Health (BH) for their health issues. You will have a strong overview of services available across BH and can direct consumers to the most appropriate services as required.
You'll be responsible, under the direction of a relevant health professional (Registered Nurse or Allied Health Practitioner), for the provision of holistic screening of referrals and enquiries, in order to achieve access to the correct services and priority for care according to the consumer's needs. Working within your scope of practice, you will identify additional services of benefit to the client and provide a range of information to the consumer about services, fees, eligibility and waiting times. As the intake worker you will liaise closely with the team to confirm service prioritisation or escalate care to ensure a safe and high-quality experience for our consumer. This role will work in partnership with Information and Access Service staff and site Clinical and Administrative staff.
Key responsibilities
* Assess referrals, identify consumer issues, and determine the most appropriate service based on clinical need and eligibility.
* Provide care and support within own competency level, in line with best practice standards and Barwon Health requirements.
* Use clinical and discipline-specific tools to prioritise clients, identify urgent cases, and respond appropriately.
* Communicate details about services, eligibility, waiting times, and fees, using effective communication frameworks such as AIDET.
What we're looking for
* Registration with AHPRA as an Enrolled Nurse Division 2
* Cert IV Allied Health Assistant
* Demonstrated significant post graduate experience
