About the role
The Chronic Conditions Nurse (CCN) plays a critical clinical and leadership role as part of an innovative 12-month initiative to strengthen multidisciplinary team based (MDT) care for individuals with chronic and complex conditions in Kununurra.
This position will uplift systems and process and deliver hands on care to ensure individuals receive person-centred, coordinated, supportive and culturally safe healthcare.
Working collaboratively with the Practice Manager, Clinical Director and the broader Senior Management Team, this role is responsible for supporting GPs with GP Chronic Condition Management Plans (GPCCMPs) and GP Mental Health Treatment Plans (GPMHTP) to enhance access to timely, multidisciplinary allied health and mental health care.
This position also plays an important role in supporting the seamless integration of our new allied health program into our model of care.
In partnership with the (new) Kununurra Integrated Care Clinical Action Group (KICCAG), and key stakeholders, the CCN will strengthen and develop integrated pathways to improve inter-agency coordination and delivery of MDT care across Kununurra healthcare services.
The CCN will provide leadership and direct supervision for the Aboriginal Health Practitioner (AHP) (Chronic Conditions) to ensure services are culturally responsive and sensitive to local contexts so that individuals feel safe and supported throughout their healthcare journey.

Experience and Qualifications required
Tertiary qualifications nursing;
Full registration with AHPRA as a registered nurse;
Minimum of 3 years postgraduate work experience;
Demonstrated clinical expertise in complex care coordination, MDT, and person-centred chronic conditions management;
Well-developed written, verbal and interpersonal skills including the ability to communicate sensitively and effectively in a cross-cultural environment;
Demonstrated ability to form effective professional relationships with a wide range of stakeholders, including the ability to engage in constructive negotiations;
Strong understanding of clinical governance, contract compliance and program evaluation;
Strong leadership skills with ability to motivate, mentor and inspire a multidisciplinary team towards program and organisational goals;
Proven ability to thrive in a fast-paced, complex medical, private practice or primary health care environment;
Sound knowledge of Work, Health and Safety legislation, standards and codes of practice;
Proficient digital and IT skills including electronic health record systems and Office 365 suite of applications.
