* Salary Range: $90,235 - $103,724 + superannuation
The Ambulatory Aged Care Program (AACP) consists of 3 programs: Transition care program (TCP), Restorative Care Program (REST) and GEM in The Home (GITH). These services aim to restore client health and wellbeing through a multidisciplinary, time-limited and goal-oriented approach. Physiotherapy services are client-focused, efficient and meet established standards of excellence. The PT team in AACP has a varied caseload and works closely with AACP Team Assistants to provide regular PT interventions for all clients. The Grade 2 PT actively participates in the clinical supervision and education of Grade 1 PTs and students, and supervises and mentors Team Assistants working in AACP.
This is an ideal role for an experienced PT who enjoys working independently as part of a multidisciplinary, community-based team and who has excellent assessment skills to tailor individualised, patient-centred interventions to optimise client outcomes.
Responsibilities
* Work autonomously in a multidisciplinary, community-based team
* Provide interventions in predominantly home-based settings
* Undertake a varied and diverse workload and tailor interventions to improve independence and well-being
* Contribute to clinical supervision and mentoring within the AACP
Additional details
* Recruitment bonus of up to $5,000 paid after six months probation (GV Health only)
* Permanent relocation support: Intrastate up to $2,500; Interstate up to $5,000; Overseas up to $8,000
* Accommodation support: Up to $350 per week for up to 10 weeks, or 10 weeks interim accommodation (if available)
