The Opportunity We are seeking an experienced and motivated Nurse Practitioner to join our client's innovative and committed Residential In Reach Aged Care Services team. This is a part-time to full time role offered on a fixed-term contract for 24 months. The Role As an In Reach Nurse Practitioner, you will: Provide advanced clinical care, including assessment, diagnosis, treatment and management of residents’ acute and chronic conditions in consultation with the GP and the multidisciplinary team. Prescribe medications and order/interpret diagnostic tests in line with professional scope and legislative standards. Develop and implement individualised care plans in collaboration with residents, families, and the multidisciplinary team. Lead, mentor and support nursing staff to promote best practice aged care. Ensure compliance with clinical governance, quality standards and safety protocols. Advocate for residents’ wellbeing, independence and quality of life while promoting health education and preventative care. Collect and analyse RIR data and KPI’s The Best Bits Excellent salary packaging options, including meals and entertainment and novated leasing Be part of a high performing team Free car parking Access to internal and external professional training and development opportunities Supportive culture that promotes staff health and wellbeing About You To be successful in this position, our ideal candidate: Hold current Nurse Practitioner registration with AHPRA and relevant endorsements. Have extensive experience in aged care, including managing complex and chronic health conditions. Relevant post graduate qualifications in aged care, health care or a related field. Is proficient in all Microsoft Office applications. Ability to work collaboratively and cohesively with colleagues, supervisors, and other stakeholders Current Driving Licence Willingness to travel Please note: Successful applicants will need to hold, or be able to obtain, a current National Criminal Police Check. Depending on the role, you may also be required to obtain a Working with Children Check clearance and/or an NDIS Worker Screening Check. All team members must meet our clients immunisation and vaccination requirements, including COVID-19. The Essentials Demonstrate strong clinical assessment, diagnostic and prescribing skills. Provide leadership, mentorship and support to nursing and care staff. Exhibit excellent communication, interpersonal and resident-focused care skills. High attention to detail and accuracy.
