The Opportunity We are seeking an experienced and motivated Nurse Practitioner to join our client's innovative and committed Residential In Reach Aged Care Services team. This is a part-time to full time role offered on a fixed-term contract for 24 months. The Role As an In Reach Nurse Practitioner, you will: Provide advanced clinical care, including assessment, diagnosis, treatment and management of residents’ acute and chronic conditions in consultation with the GP and the multidisciplinary team. Prescribe medications and order/interpret diagnostic tests in line with professional scope and legislative standards. Develop and implement individualised care plans in collaboration with residents, families, and the multidisciplinary team. Lead, mentor and support nursing staff to promote best practice aged care. Ensure compliance with clinical governance, quality standards and safety protocols. Advocate for residents’ wellbeing, independence and quality of life while promoting health education and preventative care. Have extensive experience in aged care, including managing complex and chronic health conditions. Relevant post graduate qualifications in aged care, health care or a related field. Is proficient in all Microsoft Office applications.
