The Patient Liaison Nurse role is responsible for the coordination of ongoing care and/or placement of aged persons, organisation and transfer of patient to outlying hospitals, clients of Disability Services and facilitation of discharge process of long stay and complex patients in the acute units.
Successful candidate will have:
- Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse
- Minimum of 3 years post graduate acute nursing experience
- Demonstrated expert clinical leadership, experience and knowledge related to health care
- Demonstrate understand of acute, subacute and community-based services
- Self-motivated with high level of management and information systems
- Computer literacy at intermediate level
- Knowledge of current statutory requirements including relevant awards, OH&S.;
