We are seeking a dedicated Clinical Care Coordinator to join our team at Lyrebird Village in Drouin, VIC, on a Full‑Time basis. Situated just over an hour from Melbourne Drouin is surrounded by rolling hills and fresh country air. It offers the perfect mix of rural charm and modern convenience. About the role

The Clinical Care Coordinator is responsible for leading and coordinating high‑quality, person‑centred care in line with best practice and organisational standards. Working closely with the Care Manager, this role ensures residents have up‑to‑date Care Plans tailored to their individual needs and oversees their implementation with the support of a multidisciplinary team. This position reports to the Care Manager or General Manager to ensure effective coordination of resident care, including:

Assist with the admission of new residents, ensuring comprehensive assessments are completed.

Assess individual resident needs and develop person‑centred care plans in collaboration with healthcare professionals.

Lead and support a dedicated care team to deliver safe, high‑quality clinical care.

Maintain accurate and up‑to‑date clinical documentation and ensure compliance with regulatory standards.

Provide hands‑on nursing care, including wound management, medication administration, and monitoring.

Communicate effectively with residents, families, and healthcare providers to support positive outcomes.

Oversee infection control measures, clinical audits, and quality improvement initiatives.

Act as a role model and mentor, supporting and developing nursing and care staff.

Essential Requirements

We are looking for someone who brings:

Bachelor of Nursing

Current Australian Health Practitioner Regulation Agency (AHPRA) registration

Nursing experience within a residential aged care facility, preferably at least 2 years of aged care experience.

Experience of working cooperatively with multi‑disciplinary care teams

Robust clinical assessment, documentation and practice skills

Effective communication and interpersonal skills

Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.

Experience in a similar clinical care role

An understanding of the aged care accreditation process

To be considered for employment with Respect, each candidate must satisfy the legislative requirements of Aged Care Worker Screening to ensure suitability to work in the aged care industry.
