**About the role**:
You will be responsible for patient discharge planning, as well as the development, co-ordination and facilitation of the Drug and Alcohol Clinical Services (DACS) Lakeview Group Program, which supports and encourages social activities, health promotion and education.
**Where you’ll be working**:
Lakeview Withdrawal management service is an inpatient unit located on the grounds of Belmont Hospital Campus. Lakeview operates with a team of Addiction Medicine Specialists, Registered Nurses, Allied Health professionals, and Administration Staff.
You will be supported by the Nurse Unit Manager and other multidisciplinary professionals within the team and the wider service, as well as the DACS Allied Health Clinical Lead. Professional development is encouraged and supported, and DACS provides education sessions regularly that you are welcome to attend.

- Must hold a qualification in Social Work which provides eligibility or membership of the Australian Association of Social Workers.
**Additional Information**:
- To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a Recent Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.
