St Agnes' Catholic Parish is a not-for-profit organisation has been serving the Port Macquarie community since 1878, offering compassionate support to people of all ages and life stages.

With over 700 staff across a range of ministries, including aged care, community and family, education and training, and ministry and pastoral, the Parish works to support the dignity, wellbeing and needs of the community.

ABOUT THE ROLE

Our Home Care service supports elderly residents to live independently in their own homes by providing a range of services tailored to their needs. These services include personal care, nursing, domestic assistance and Support At Home packages designed to empower clients to maintain their preferred lifestyle.

This role focuses on case facilitation and the coordination of client care services through a person-centered approach. The position involves working collaboratively with clients, their families and associated providers to deliver comprehensive care planning, implementation, monitoring, and review.

KEY RESPONSIBILITIES
* Developing and maintaining individual care plans with a clear focus on enablement and reablement goals.
* Facilitating effective interactions with clients, families and service providers to ensure high-quality service delivery.
* Ensuring efficient and sustainable management of client funding, including balanced budgeting and future planning.
* Advocating for and supporting clients in accessing the services they require.
* Promoting client health and well-being by conducting thorough assessments and tailoring care to meet individual needs.
* Maintaining compliance with all relevant policies, safety protocols and consumer-directed care principles.

The position will be offered to the successful applicant as a Permanent Full Time position, working 76hrs a fortnight - 5 days a week.

WHO YOU ARE

Commitment to work in harmony with Catholic Values and Principles

An energetic, positive attitude with a genuine interest in and compassion for people

Certificate III in Community services or Aged Care (Or Similar)

First Aid/CPR

Demonstrated ability to exercise initiative and judgement to problem-solve complex issues

Demonstrated experience in planning, prioritising, implementing and managing budgets

Well-developed communication and interpersonal skills, with the ability to work as part of a team

Computer & phone skills with willingness to undergo training if needed

A current Drivers Licence
