The Opportunity
As a Case Manager, you will:
* Provide tailored support to vulnerable individuals and families.
* Collaborate with clients, families, carers, and service providers.
* Assess needs, develop care plans, and monitor progress.
* Work with a caseload of young people in therapeutic care
What's Needed to Succeed:
* A diploma in Community Services or a graduate degree in psychology, social work, or related discipline.
* Experience supporting adolescents and families with trauma.
* Knowledge of inter-generational trauma and relevant legislation
