Adults Community Support Worker
We are seeking an experienced social worker to join our team. The ideal candidate will have a strong understanding of adult social care and be able to work in a fast-paced environment.
Job Description:
As a Adults Community Support Worker, you will be responsible for completing social care needs assessments, support planning, and commissioning services to meet the needs of adults with complex care requirements.
You will work closely with other professionals to ensure that individuals receive the best possible care and support. You will also be required to participate in MARACs (Multi-Agency Risk Assessment Conferences) and other meetings as necessary.
In addition to your professional skills and knowledge, we are looking for someone who is enthusiastic, flexible, and able to work well in a team.
Key Responsibilities:
- Completing social care needs assessments
- Support planning
- Commissioning
- Risk assessment and risk management, including attending MARACs
- Safeguarding
- MCA/BI (Mental Capacity Act / Best Interests)
- CAP DOLS (Deprivation of Liberty Safeguards)
- CHC – DST (Continuing Healthcare – Decision Support Tool)
Requirements:
- A degree in Social Work or a related field
- Registered with the HCPC (Health and Care Professions Council)
- At least 2 years post qualifying experience in a relevant role
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work varying shifts and hours
