About your opportunity: In this position you will * Provide comprehensive patient management within a Multi-Disciplinary Team (MDT) by providing information about hospital services, participating in discharge planning and referring to the community for services. * Deliver specific health care services as required depending on local community needs and service arrangements, and within the limits of own qualifications, experience and competency in Women's Health Management Programs. * Be self-directed and participate in activities such as in-service programs, monthly staff meetings, weekly case conference, individual/group supervision, quality programs, statistical collection/collation, audits, record keeping and report writing.
