**The role**:
As a Mental Health Practitioner, you will play a pivotal role in delivering individualized, recovery-focused, psychosocial support to individuals experiencing mental ill-health. Working in an outreach capacity across the state, you will engage with people within their communities, utilizing a 'wrap around' approach that considers both personal and relational recovery journeys. Your role emphasizes collaboration with key stakeholders, including the individuals themselves, family, carers, mental health clinicians, and other services in the care continuum. You will bring diverse experiences to a time of various disciplines, working alongside individuals to build capacity through strengths-based assessments and goal-based case plans.

A core aspect of your role will involve providing a developmental service, which includes offering relevant information, assisting in goal establishment, and continually assessing and reviewing the needs of individuals with mental health issues. You will work closely with individuals to access necessary services, promoting the improvement and maintenance of their quality of life. In this position you will require comprehensive assessment, planning, and interpersonal skills, developing goal-based case plans in collaboration with the individuals, their families, friends, and other supports. To ensure the delivery of evidence-based mental health practice, you will work under the guidance of a Senior Practitioner and actively contribute to meeting the program's Key Performance Indicators.

In addition to the direct support provided, You will contribute to the program's links with relevant organizations, facilitate coordinated and integrated services, and actively engage in ongoing learning and development. Your role also places a strong emphasis on workplace health and safety, with practitioners committed to achieving a Zero harm workplace and promoting a safety culture within the organization. Overall, you will play a crucial role in positively impacting the lives of individuals experiencing mental health challenges by fostering independence, promoting recovery, and contributing to a supportive and inclusive community.

**Essential Criteria**
- Minimum Certificate IV in Mental Health
- Cultural Awareness Training
- Minimum 1 years' experience in Mental Health
- Experience working in outreach settings
- Inclusion Training
- Working with Children's Check
- Driving license
