St John of God Bunbury Hospital is currently seeking a Discharge Nurse to join our team in a permanent part time position. Working collaboratively with internal and external multidisciplinary stakeholders to facilitate safe, appropriate and timely discharge planning for patients with complex needs.

In conjunction with the Discharge Coordinator and relevant Nurse Unit Managers, the role supports discharge planning across a diverse patient cohort, including DVA clients, while contributing to the ongoing evaluation and improvement of discharge processes to enhance quality, continuity of care and patient outcomes. The Position

* Actively coordinates the discharge of patients assessed as requiring a Discharge Planner.
* Ensures that early discharge planning is commenced in line with the appropriate length of stay.
* Coordinates discharge planning processes through liaison with other health professionals to ensure the effective discharge of patients throughout the hospital.
* Ensures patients and carers are informed and participate in the discharge planning process and the available support options.
* Is active in communicating with other service providers regarding "at risk" patients, who may require coordination of multiple services.
* Assists nursing management in the coordination of clinical care as it pertains to length of stay, and patient‐centred care.
* Ensures patient confidentiality whilst maintaining accurate and objective records of care provided.
* Acts as a resource to medical and ward staff to assist in the identification of patient needs pertaining to discharge.
* Displays knowledge of the health fund agreements as appropriate to the Discharge Planning role.
* Acts as a community liaison and resource person by providing information and advice to clinical caregivers, patients, carers, relatives and other service providers in the region.
* Is proactive in continually evaluating and improving the discharge planning service.
* Actively participates in quality improvement activities, to ensure optimal clinical standards are maintained and is proactive in promoting enhanced patient outcomes.
* Liaises with the Discharge Planning Coordinator to effectively manage LOS for patients identified as complex or at risk for extended LOS.
* Acts to provide a service that is safe, efficient, cost effective and of the highest quality.
* Collects data and maintain database of patients assisted by Discharge Planning Service.
* Is active in assisting to reduce overall patient length of stay while maintaining a quality service and the SJGHC Mission and Values.
* Demonstrates a commitment to personal and professional development of yourself and other staff members.
* Continually reviews own work practices with a view to continuous improvement.

To be successful in this role, you will be a registered nurse with AHPRA or bring other relevant experience within a health‐related field, supported by a minimum of four years' current acute clinical experience or recent experience in discharge planning. You will have a strong understanding of discharge planning processes and demonstrated experience working effectively within an interdisciplinary healthcare team.

You will be a self‐motivated and reliable professional with the ability to work independently, using initiative and requiring minimal supervision. Strong communication skills are essential, enabling you to engage effectively with patients, families, caregivers, VMOs and external service providers to support safe, coordinated and timely discharge outcomes.

* Salary: $109,599 to $115,682 pro rata for part time, plus 12% superannuation
* Permanent part time position, working 53.2 hours per fortnight
