Your Role As a Clinical Aged Care Assessor, you'll play a crucial part in identifying the needs of older adults and connecting them with services that promote their independence and overall well-being.
Main Responsibilities:
* Conduct comprehensive Aged Care Assessments to determine the requirements of older adults
* Coordinate referrals to essential services supporting independent living
* Promote the retention and maximization of function and capability in daily activities
* Work closely with families and carers, acknowledging them as key stakeholders
* E nsure all assessments are conducted in accordance with Department of Health contractual specifications
Key Requirements:
* At least 1 year of experience in aged care, including client assessments and/or service delivery
* Tertiary education in a health-related field (e.g., nursing, occupational therapy, physiotherapy, social work)
* Current unrestricted General Registration with AHPRA or relevant professional association
* Proficiency in Microsoft Office applications (Word, Excel, Outlook) and client management systems
* Australian driver's license, comprehensively insured vehicle, and ability to travel within your designated service area
