Your new role: In partnership with the Nurse Unit Manager, you will provide operational leadership within the Transit Care Hub to support safe and timely discharge of sub-acute patients by: * Leading and coordinating overnight and early discharge planning to optimise patient flow * Collaborating with treating teams and multidisciplinary services to identify suitable patients for transfer to Transit Care Hub * Providing clinical and professional leadership to nursing staff, ensuring safe, person-centred care aligned with best practice * Supporting the utilisation and accuracy of Patient Flow Manager (PFM), including Estimated Discharge Dates (EDD) * Contributing to performance monitoring, data collection and reporting against key patient flow indicators * Supporting quality improvement, change management and service development initiatives within the Patient Flow Program About you: You are an experienced and motivated Registered Nurse with a strong understanding of patient flow and discharge processes, who brings advanced clinical judgement, leadership capability and a collaborative approach to care by: * Holding current registration with AHPRA as a Registered Nurse * Demonstrating advanced clinical knowledge in discharge planning, patient flow or sub-acute care * Communicating effectively with multidisciplinary teams to negotiate and coordinate complex discharges * Applying critical thinking and problem-solving skills to manage competing priorities in a dynamic environment * Leading, mentoring and supporting nursing staff to deliver safe, high-quality, person-centred care * Using data, systems and evidence-based practice to support service improvement and performance outcomes * (Postgraduate qualifications or enrolment in a relevant program will be highly regarded.)
