The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.
About you
This position provides a quality social work service to Aboriginal people in the South West region (with possible support of our Bindjareb department) that is culturally appropriate, secure, and respectful.
You will be part of a multidisciplinary team consisting of OT/PT/RN/CSW delivering case-management, advocacy, support, and capacity building services to SWAMS clients, particularly those engaged with our Community Services Department, such as our Aged Care programs and NDIS.
The main duty of the Community Services social worker is to provide culturally secure case management for clients and their family members through the provision of advocacy, follow-up, referral to appropriate services, and collaboration with other support agencies. This role mostly works within the Transition Care (TCP) team, facilitating a smooth hospital discharge for our clients by completing necessary TCP on-boarding documentation to ensure quick transition from hospital to necessary community services, whilst also assessing for any on-going Social Work needs and goals.
Selection Criteria:
* Tertiary qualification in Social Work eligible for full membership of the Australian Association of Social Workers (AASW) or the Society of Professional Social Workers (SPSW).
* Experience in providing culturally appropriate support services, including case management, counselling, advocacy and supports to Aboriginal people.
* Demonstrated understanding of privacy and confidentiality issues.
* Demonstrated effective communication and interpersonal skills
* Ability to function independently or as part of a multidisciplinary team
* Demonstrated high level of written, interpersonal and communication skills.
* Ability to be flexible and manage case load and time effectively
* Ability to travel throughout the South West and Bindjareb regions.
* Valid and current Western Australian Driver's License
* Satisfactory Criminal History Check
* Valid Employee Working with Children Check
Desirable Requirements:
* Experience and knowledge of the Aged Care sector, particularly with HCP, CHSP, TCP 2.
* Experience and knowledge of the NDIS sector.
* Aboriginal or Torres Strait Islander descent.
* Understanding of the inter-generational impacts of Stolen Generation policies.
