About the Role
An exciting opportunity exists for a Diabetes Clinical Nurse Consultant to gain experience in our HARP Diabetes Team. You will be providing diabetes self‐management education to people living with diabetes who need extra support to manage their condition, including attending outpatient clinics, face‐to‐face consultations, phone consultations and home visits. If you like a challenge, this is the opportunity for you. You will be primarily located at the Footscray campus.
Key Responsibilities
* Support the diabetes clinic by seeing patients face‐to‐face, via telehealth, telephone and performing home visits (if required)
* Support patients living with diabetes to achieve their best outcomes
* Work collaboratively within the team
About You
* Tertiary qualification in Diabetes Management
* Self‐motivated and committed to lifelong learning
