The Pharmacy Department at the Mackay Base Hospital is seeking applications for a Senior Pharmacist (Specialist Palliative Care Service) to join their team on a permanent basis.

Your opportunity
To deliver high level comprehensive Pharmacy clinical skills and leadership to provide effective and efficient treatment to patients and their families engaged with the Specialist Palliative Care Service (SPCS) multi-disciplinary team. Clinical service provision will be to patients, their families/carers and other key stakeholders via home visits, outpatient appointments, inpatient appointments and telehealth.
About The Team
The MHHS Specialist Palliative Care Service (SPCS) aims to improve the quality of life for patients and their families facing the problems associated with life-limiting illness. This can be achieved through the prevention and relief of suffering by means of early identification and treatment of pain and other problems, in the physical, psychological and spiritual domains. The SPCS seeks to:
* Provide holistic patient‐centred care for patients with life-limiting illness across the care continuum.
* Provide specialist palliative care for patients with life-limiting illness which is needs-based regardless of diagnosis or prognosis.
* Address the specialist palliative care needs of patients and their families for the duration of their illness trajectory.
* Provide multidisciplinary care in a reliable and efficient manner.
