Senior Care Nurse
We are seeking a dedicated Senior Care Nurse to join our Interprofessional Primary Care Team.
The successful candidate will work independently at full scope of practice, focusing on Seniors Health. They will support older adults and their caregivers through the aging journey, conducting comprehensive geriatric assessments and developing individualized patient-centered care plans.
* Conduct comprehensive and focused nursing assessments based on individual concerns and complexity
* Knowledge of different types and stages of dementia and cognitive impairment, following current best practice guidelines
* Client-focused orientation with a commitment to providing quality services and programs
Candidates must have an active Practice Permit with CARNACurrent BLS/CPR Certification and 3-5 years' experience in nursing. Experience in community-based care is an asset, and Geriatrics experience is preferred.
