The Role: An exciting opportunity has arisen for a Client Service Coordinator to join the team for a 12-month contract (maternity leave cover).
This role offers a chance for someone with a background in case management, client care, and the health and disability sector to make a meaningful impact.In this role, the successful candidate will: Coordinate client care and support the development and implementation of personalised care plans.
Manage client case management, including addressing complex needs and ensuring the delivery of quality services.
Supervise, mentor, and coach staff to maintain a high standard of care.
Handle complaints, incident management, and reporting, ensuring compliance with relevant policies and procedures.
Work with clients to support goal setting and care planning activities, promoting their independence and empowering decision-making.
What the Organisation is Looking For: Qualifications: Tertiary qualifications in health or a related field (e.g., Human Services) are desirable.
Diploma in Disability or a related discipline is preferred.
Current First Aid Certificate.
NDIS Workers Screening Check and National Police Clearance.
Working With Children Check.
Current Driver's License.
Experience: Sound knowledge of the NDIS framework and Home Care Packages funding models.
Previous experience in case management and client care coordination, particularly in the disability or related fields.
