About the Job:
* This role involves providing goal-oriented support plans and care coordination services to empower clients to achieve stability and independence.
* You will be responsible for developing and implementing holistic support plans, coordinating access to mainstream and specialist services, and promoting Trauma-Informed Care and Practice principles.
Your Key Responsibilities:
* Develop individually tailored support plans to promote client stability and independence
* Coordinate access to essential services, including specialist services where required
* Promote a trauma-informed approach in all interactions with clients
* Provide high-quality, client-centered support to ensure optimal outcomes
Requirements:
* Diploma of Community Services or equivalent tertiary qualification
* In-depth knowledge of homelessness causes and systems, including relevant research and best practices
* A valid Victorian Driver's Licence and Working with Children Check are mandatory
