Clinical Care Coordinator

For all people in Australia living with mental health and wellbeing challenges to live a life that is meaningful to them.

Position Details

* Part Time Permanent Position (0.8FTE)
* Classification: Registered Nurse Level 2 with salary range of: $100,064 - $105,123 per annum pro rata + Superannuation OR
* Classification: Health Professional Level 2 with a salary range of $94,927 - $106,210 per annum pro rata + Superannuation
* Salary packaging arrangements providing tax benefits available for living and entertainment expenses increasing your take home income where part of your wage is paid tax free each pay, savings thousands in tax each year
* Location: Port Lincoln, SA 5606

About your newrole

The position is funded through Country SA PHN to improve access and provide adequate level of primary mental health care intervention to maximise recovery and prevent escalation. The aim is to provide wrap around coordinated care for people living in Port Lincoln and surrounding areas.

The focus of this program is provision of services for eligible people with sever mental illness and complex needs. Coordinate support between services including GP's and state funded Local Health Networks and referrers to help match people to the service pathway which best meet their needs.

What you will bring to the role

* Formal tertiary qualifications in Nursing, Psychology, Social Work, Occupational Therapy or related
* Current full registration as an Enrolled Nurse/Registered Nurse/Nurse Practitioner with current annual practicing certificate from the Australian Health Practitioner Regulation Agency (AHPRA), OR
* Current full registration as a Psychologist, Occupational Therapist or Registered Nurse with current annual practicing certificate from the Australian Health Practitioner Regulation Agency (AHPRA), OR
* Current full registration as a Social Worker with the Australian Association of Social Workers (AASW), or be eligible to hold membership with the Australian Association of Social Workers (AASW)
* Formal qualifications in the following disciplines: Mental Health Social Work or Mental Health Nursing (or working towards accreditation to become a Mental Health Social Worker)
* Accredited registration with a professional governing body such as AHPRA, ACA, AASW
* Experience in the area of mental health
* Experience in delivery of evidence-based brief interventions

What the role requires

* National Criminal History Check (police check) - disclosable outcomes considered
* Current Working with Children Check - required before commencement of work
* Australian working rights
* A valid driver's licence
