**Our Journey**
The Aged and Community Care (ACC) Program provides an interdisciplinary service to support older people in the community. The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital or residential care.
We have an opportunity for an enthusiastic Grade 2 Dietitian to take up a permanent part-time role, working 3 days between Monday to Friday. Grade 1's looking for the next step in their career strongly encouraged to apply!
**A Day in the Life**
The Grade 2 Dietitian will work collaboratively within a multidisciplinary team to support clients to promote independence and to remain living at home and within the community for as long as possible. The role requires provision of evidence-informed assessment, intervention and discharge planning to clients within the Aged and Community Care Program.
Dietitians work with clients and/or carers to provide nutrition care in the community and this can be in the home, onsite and/or by telehealth, as well as group programs. They will actively contribute to the Nutrition and Dietetic and multidisciplinary teams by providing best practice clinical care, sharing knowledge and participating in teaching and supervision of students (undergraduates and/or postgraduates), mentoring and/or supervision, research and quality project activities, and administrative responsibilities and portfolios as delegated.

- Enthusiasm, compassion & ability to work collaboratively
- An approved undergraduate degree in Nutrition and Dietetics from a Dietetics Australia (DA) accredited course or an approved Bachelor of Science Degree (or equivalent qualification) followed by a postgraduate degree in Nutrition and Dietetics from a DA accredited course or The DSR dietetics examination for overseas trained dietitians, administered by DA.
- Eligible for membership of Dietitians Australia and Accredited Practicing Dietitian (APD)
- Current Driver's License
