Purpose of role

The RN Community Nurse with Creative Respite and Community Services (CRCS) will provide high quality care and support to adults and children who require support through NDIS/DVA/Aging and Elderly/Child Safe models. Direct provision or care and services will take place in the community, in home or respite settings.

The Community Nurse is responsible for providing support and skilled care to people in their own homes and communities. The Community Nurse will work with individuals and their families to develop and implement support plans that meet their individual needs. The nurse will also provide education and training to individuals and their families.

Core Objectives Include

● Support the management team at CRCS to develop and deliver community nursing services.

● Provide support, education, and guidance to a team of nurses and support workers who report directly to the Clinical or Operational Supervisor.

● Assesses, plans, implements, and evaluates nursing care and support plans in participation with service users according to accepted nursing practice and standards.

● Works in consultation with other health professionals and team members.

● Provides interventions, treatments, and therapies such as medications, and monitors responses to treatment and care plans.

● Promotes health and assists in preventing ill health by participating in health education and other health promotion activities.

● Providing information and training to service users and families about planning, treatments, and care.

● Supervises and coordinates the work of enrolled nurses and other care workers.

● Provide direct support and assistance to service users as required. The support required and is not limited to:

* Personal care support.

* Manual handling tasks.

* Meal time preparation and support.

* Medication administration and support.

* Maintaining and implementing OHS/WHS

* Assist with personal development activities.

* Provide nursing care and assessments within scope of practice

* Support service users with community engagement

● Complete reports and other documentation as required.

● Comply with all policies and procedures including but not limited to OHS/WHS Policies and Privacy and Confidentiality.

● Be committed to ongoing individual development and training

● Good interpersonal, written, and verbal communication skills.

● Reliable and empathetic.

Other duties

Fulfil other duties as required by management and other department personnel as requested/required.

PERSON SPECIFICATION

Qualifications

● Minimum undergraduate degree in nursing. Additional specialty studies in an area of interest such as infection prevention and control, OHS, mental health, management, critical care, aged care, stomal therapy, community or other relevant areas will be highly regarded.

Experience

● Minimum 3 years of experience

Knowledge

● Good understanding of principles underpinning the disability, aged care and mental health sectors.

Skills & competencies

● Excellent Communication: the ability to communicate clearly and concisely, varying communication style depending upon the audience.

● Attention to Detail: excellent attention to detail and written skills when communicating with others, both internally and externally.

● Effective and Professional Teamwork: willingness to assist and support others as required and work effectively as a team member.

● Effective Time management/organisational skills: accomplish objectives effectively within the time frame given and carry out duties within job description in an efficient and timely manner. Ability to manage time, prioritising and allocation of duties, cares and tasks.

Personal attributes

Other

● Ability to work varying hours or shifts.

● AHPRA Registration

● Work may require a Blue Card

● Professional Indemnity Insurance

● QLD Open Driver's license.

● Current First Aid and CPR

● Must be eligible to work in Australia.

● Police Check and or other relevant field checks

● Experience with complex care needs.

● Experience in supervising newer nursing grads/students

● Excellent time management skills with demonstrated ability to plan and

coordinate multiple tasks.

● Excellent communication, interpersonal and problem-solving skills

Job Type: Part-time

Pay: $42.03 – $48.83 per hour
