About the role
The CFH RN/RM will provide quality child and family health services to families caring for children 0‐5 years of age via telephone and virtual consultation. The role operates daily from 7:00 am to 11:00 pm, 7 days a week, with a rotating roster and no night duty.
Responsibilities
* Deliver strength‐based, wellness‐focused support to parents, building confidence and skills through information, education, resources and role‐modeling safe responsive care.
* Use evidence‐based screening and assessment tools to identify health variations or family vulnerabilities.
* Partner with families to negotiate care plans, discharge pathways and referral services within the community.
* Provide services via telephone, telehealth, virtual consultations and online interaction, following the Tresillian Model of Care and NSW Health Professional Practice Framework for Child and Family Health Nursing.
* Work collaboratively with a multidisciplinary clinical team.
Qualifications
* Current registration as a Registered Nurse and/or Registered Midwife (AHPRA).
* Post‐graduate or post‐registration qualifications and relevant experience in Child and Family Health.
* Commitment to evidence‐based practice.
* Experience attending to the physical, social and emotional needs of children and their families.
* Knowledge of population health and primary health care principles.
* Demonstrated skills in partnership working and aligning with NSW SafeStart principles in a multidisciplinary environment.
* Effective verbal and written communication, negotiation skills and computer literacy meeting National Informatics Standards for Nurses and Midwives.
* Valid NSW Driver's Licence Class C.
