About the Role
We’re looking for a committed Aboriginal Health Worker/Practitioner to join our team, providing a vital link between Aboriginal clients and health services. You’ll work across both clinical and community settings, supporting Aboriginal clients aged 17 and over through wellness checks, case coordination, and culturally safe care delivery.
Responsibilities
- Deliver effective, protected practice within your assessed level of competence, aligned with the NSW Health Aboriginal Health Worker Guidelines and NSW Health Decision Making Framework.
- Provide primary health care in partnership with the multidisciplinary team, meeting the physical, emotional, cultural, social, and environmental needs of Aboriginal clients.
- Plan, develop, implement, and review Aboriginal health programs, including health promotion, education, and disease prevention initiatives, ensuring they are culturally appropriate and community-driven.
- Apply your knowledge of local Aboriginal communities, ensuring health care delivery is holistic, culturally safe, and responsive.
- Ensure accurate documentation, data collection, and record management in line with policies and procedures, maintaining continuity and quality of care.
- Provide mentoring and cultural guidance to colleagues, supporting cultural awareness and the orientation of staff across the District.

Requirements
- Be willing to work towards obtaining a Certificate IV Aboriginal Primary Health Care (Practice) within 2 years of enrolment in course and commencement of employment and work towards AHPRA standards to meet Registration as an Aboriginal Health Practitioner.
- Current NSW Drivers Licence and the ability to travel within the local area and throughout Hunter New England NSW LHD (as necessary).
