$89739 - $97129 p.a+Salary Packaging up to $15,900

Responsibilities

* Work independently within your region, conducting holistic assessments with a client‐centred approach focused on wellness and reablement.
* Capture individual client information and collaborate to develop their support plan and unique goals.
* Support older Australians to continue living independently, considering assists such as assistive technology, social support services, and community care.
* Utilise the integrated assessment tool with My Aged Care.

Qualifications

* Experience in assessment and goal‐oriented support planning in aged, community, health or social services.
* Confident communicator with a positive approach when collaborating with clients, families, staff, and other stakeholders.
* Local knowledge and the ability to network with stakeholders within your region.
* Advanced IT skills with experience across multiple platforms (Microsoft Office Suite).
* Previous experience working with KPIs to ensure high quality client outcomes.
* Ability to manage your time effectively in an autonomous working environment.

What to Bring

* Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with AASW Registration.
* Minimum one year experience within aged care.
