As a **Care Finder Support Worker**, you will provide specialist and intensive assistance to older people who need extra help navigating the aged care system. You'll work in the community, building trusted relationships, and ensuring people are supported to access aged care services and other relevant community support. **month, part-time position with a total commitment of 19 hours per week, to be worked across Tuesday, Wednesday, and Thursday. The role is based in the Lower Blue Mountains region (Penrith office).**In this role, you will support people within the Care Finder target population by engaging through assertive outreach, building trust, and guiding them through My Aged Care and related service pathways.
