Looking for a new challenge in healthcare? We have an exciting opportunity for a Clinical Aged Care Assessor to join our team. In this role, you will be responsible for conducting comprehensive assessments of customers seeking home support services. Using the Integrated Assessment Tool (IAT), you will evaluate and determine the needs of our customers, developing personalised support plans and connecting relevant service providers to enable them to retain their independence.
The successful candidate will be working in a permanent, full-time capacity, Monday to Friday, with travel required within the Hervey Bay region. However, we also offer flexible options for part-time or casual arrangements with a minimum of 3 days per week or reduced daily hours across 4 or 5 days.

Key Responsibilities
* Conduct comprehensive assessments using the IAT to evaluate and determine the needs of customers
* Develop personalised support plans and connect relevant service providers
* Collaborate and communicate with customers, carers, families, representatives, and other service providers
* Undertake and review personalised support plans, effectively managing changing customer and carer needs
* Identify customers' formal and/or informal respite support and arrangements
* Develop Emergency Care Plans if necessary
* Referring for services, assessing complexity and risk of vulnerability, and assisting with linking support, reablement, and short-term case management
* Participate in clinical supervision and undertake various administrative functions, including report writing and correspondence preparation
Requirements
* Tertiary qualification in a health-related discipline (e.g., Registered Nurse, Physiotherapist, or Social Worker)
* Current unrestricted AHPRA registration or eligibility to register with a relevant professional association
* Minimum one year's experience directly delivering services in aged care settings and/or to aged persons
* Knowledge or experience in conducting home assessments or risk assessments for vulnerable people
* Demonstrated ability to communicate clearly and effectively with customers with hearing loss, sensory loss, or cognitive impairment
* Experience in undertaking assessments of customers with dementia who have responsive behaviours
* Adaptability and flexibility to respond to changing task requirements
* Willingness to use own registered and insured vehicle for work purposes and hold a valid Driver's Licence
* Current First Aid and CPR Certificate, and evidence of most current influenza vaccination
