We offer specialist palliative care services to individuals with life-limiting illnesses, empowering them to live well and maintain their dignity.
This role requires advanced clinical decision-making skills, problem-solving abilities, and a solution-oriented mindset. As a skilled palliative care nurse, you will contribute to upholding high clinical standards while delivering compassionate care to clients and their families/carers.
You will play a critical role in coordinating complex care arrangements and managing cases, collaborating with external stakeholders and community service providers. This position involves rotational duties, including access, intake, and responding to clinical escalations to ensure seamless service delivery.
As a positive influence and mentor, you will guide less experienced colleagues, fostering skill development and professional growth. You will lead within the clinical team, driving quality improvement initiatives and research projects.
 * Current registration with Australian Health Practitioner Regulation Agency (APHRA)
 * Demonstrated ability to provide coordinated care in partnership with key external service providers
 * Proficiency in Microsoft Office suite and clinical electronic databases
 * Excellent interpersonal and communication skills
