Reporting to our Clinical Nurse Coordinator, this is a full-time, ongoing position (1.0 FTE) however, we know flexibility matters so we'd love to hear from candidates interested in a part-time job share (0.6 FTE + 0.4 FTE) so you can make the role work for your lifestyle. Conduct initial health and social needs assessments, care planning, and case conferencing for newly arrived individuals and families.
Coordinate client healthcare by liaising with services, schools, and a range of providers (GPs, dental, nutritionists, school nurses, trauma counselling, hospitals, etc.), including facilitating access to interpreters and transportation support.

AHPRA Division 1 Registered Nurse, with full and unrestricted registration.
Minimum of 2-3 years' experience in primary health care settings or as a Refugee Health Nurse.
