Role

THE ROLE

The Health Services Coordinator delivers non‐clinical coordination of Comprehensive Health Assessments (CHA), follow‐up care, and health navigation support for newly arrived humanitarian entrants. This role builds the capacity of local health services to deliver culturally safe care and contributes to health promotion activities for multicultural communities.

Key responsibilities

* Triage HAPlite referrals and coordinate CHA appointments for new arrivals.
* Monitor client progress, follow up clients who did not attend scheduled appointments and ensure completion of CHAs and related referrals.
* Support clients at appointments when barriers or complex needs arise.
* Participate in shared decision‐making, case conferences and occasional home visits.
* Coordinate health orientation and health promotion sessions.
* Facilitate outreach clinics (optometry, audiology etc.) and maintain partnerships.
* Work Melaleuca staff and providers, identify and close health service gaps.

2. Relationship management

* Represent Melaleuca in local and national multicultural health forums.
* Build partnerships with health providers to support positive client outcomes.
* Engage with funders on compliance and service quality.
* Support capacity building of health professionals in refugee health.

3. Contracts, systems, reporting and administration

* Manage workload, maintain high‐quality service delivery, and ensure KPI/compliance requirements are met.
* Contribute to reporting, quality improvement and monitoring of health trends.
* Support the development and maintenance of best‐practice systems.

4. Organisation wide

* Comply withMelaleuca policies,proceduresand values, and positivelyreflect the organisation at all times.

* Participate in supervision and debriefing asrequired.
* Participate in essential and non-essential Professional Development (PD) andmaintainpersonal records of all PD to ensure ongoing upskilling.Participate in performance reviews on a regular basis with Manager.
* Attend meetings, training, toolbox, team building and other client related forums that may occur outside of workhours, and as required by the organisation.
* Actively contribute to the continuous quality improvement of the organisation.
* You maybe requiredto work across any other Melaleuca sites in line with your appointed role and/or skills; and undertake other duties asrequired.

Selection criteria

ESSENTIAL

* Registered Nurse with current AHPRA registration with 5+ years' experience.
* Experience working with refugees, migrants or trauma‐impacted communities
* Strong knowledge of primary health care and referral pathways.
* High‐level communication, networking, education, and health promotion skills.
* Strong record‐keeping analysis and report-writing ability.
* Ability to work autonomously, prioritise workload, and collaborate in a dynamic environment.
* Current First Aid.

DESIRABLE

* Experience working with CaLD groups and knowledge of the barriers CaLD communities face in accessing health services in Australia.
* Postgraduate qualifications in public health or health promotion.
* Experience using translating and interpreting services.
* 1. Current AHPRA registration
* 2. Current class c driver's licence
* 3. Current NT Ochre Card (Working with Children clearance)
* 4. Right to work in Australia
* 5. Satisfactory national criminal history report
