About the Role
We are seeking a passionate and experienced Clinical Team Leader – Practice / AHP to lead our clinical team in delivering high-quality, culturally appropriate care.
This role is central to supporting our community-focused model and ensuring best practice standards across all clinical services.
Key Responsibilities
Lead and mentor the nursing team to provide culturally safe, patient-centred care.
Coordinate chronic disease management, preventive health programs, and health checks tailored to community needs.
Support clinical governance, accreditation, quality assurance tasks and continuous quality improvement initiatives.
Work collaboratively with Aboriginal Health Workers, GPs, and allied health professionals.
Promote health literacy and empower clients through education and engagement.
Ensure compliance with infection control, medication management, and clinical documentation standards.
About You / Key Selection Criteria
Demonstrate knowledge of and compliance with all relevant legislation and common law obligations affecting nursing.
Discharge of duty of care during practice including meeting practice standards, and accountability.
Experience in a similar role as a practice nurse or in community-based nursing.
Knowledge and understanding of the health issues that face Aboriginal people and their communities.
Respect, understand & acknowledge the traditions of Aboriginal people.
An understanding of an Aboriginal Community Controlled Health Organisation.
Knowledge of current immunisation schedule, diabetes management, asthma management, enhancing primary care items, infection control, CPR and emergency resuscitation techniques, equipment & drugs.
Demonstrated patient-focused approach in service provision with genuine empathy and interest in their needs.
Maintain absolute confidentiality regarding patient and practice information.
Ability to effectively and efficiently plan and organise own workload.
Work cooperatively within a team as well as be able to work independently.
Demonstrated ability to manage stressful situations and demanding workloads.
Effective interpersonal skills including verbal and written communication.
Advanced computer skills, knowledge of Best Practice software.
Demonstrate previous experience in leadership and managing a team.
Education, Training and / or Competencies
Mandatory
Registered Nurse Division 1 with current AHPRA certificate.
Certificate IV Aboriginal and Torres Strait Islander Primary Health Care Practice.
Training or experience in Medicare items numbers.
Previous Cultural Awareness training, or willingness to undertake course.
Local connections to the Aboriginal and Torres Strait Islander community.
Leadership training.
