About the Role
We are looking for a Community Support Worker to join our Individualised Community Living Strategy (ICLS) program providing outreach support to a people accessing the servicein Harvey as well as surrounding suburbs including Australind.
The service is for clients with a mental health diagnosis who are eligible for a support package.
The role of the Community Support Worker is to help the client access their local services and maximise their recovery success.
Successful candidates will have previous experience working 1:1 with people with mental illness or psychosocial disability strong communication skills and confidence in working independently within the community.
Key Responsibilities
Provide dedicated support to clients with a mental health diagnosis focusing on his recovery and community living goals
Coach and encourage independence through daily routines and decision-making
Assist with cooking cleaning and home upkeep empowering the client to take control of their daily living
Encourage community access and engagement in local activities
Support capacity building and self-management of lifestyle and wellbeing
Work collaboratively with clinical and mental health support teams
Follow and contribute to individualised support plans
Skills & Experience
Certificate IV in Mental Health or equivalent
Demonstrated experience working autonomously with people with mental illness or psycho-social disability
Passionate about client outcomes and mental health advocacy
High computer literacy and administration skills
An unrestricted Western Australian drivers licence and access to a reliable vehicle
A current First Aid and CPR Certificate
Successful candidates will be required to clear all state-based probity checks; including but not limited to a National Criminal History Record Check and NDIS Worker Screening Check.
