About the Role
As a Nurse Practitioner with Monash Health Community, you will play a vital role in delivering a proactive, person-centred model of care that helps people with chronic and complex conditions stay well and safely at home. This is an exciting opportunity to use your advanced clinical skills to make a meaningful difference across both acute and community settings.
In this role, you will work across the Emergency Department and Community programs, helping to divert suitable clients away from unnecessary hospital presentations and supporting early discharge pathways. By providing timely assessment, advanced clinical interventions, and integrated care planning, you will help reduce exacerbations of chronic disease and improve long-term health outcomes.
You will be part of a dynamic service model that blends time in the Emergency Department with clinic-based care and assertive outreach, including home visits within the Monash Health catchment.
Operating within a person-centred care framework and aligned with the social model of health, you will play a central role in assessing and managing client wellbeing, empowering individuals to actively participate in their health journey.
About You
You are a highly motivated and dynamic Nurse Practitioner Candidate or Nurse Practitioner with a genuine passion for supporting people living with chronic and complex conditions. You're committed to improving the lives of some of the most vulnerable members of our community, particularly those with Chronic Heart Failure and Chronic Respiratory Disease.
You bring a strong blend of autonomy and teamwork, with the proven ability to work both independently and collaboratively across diverse clinical environments. Building meaningful relationships with clients, families, and key stakeholders comes naturally to you, and you take pride in delivering care that is truly person-centred.
You communicate with clarity and confidence—both in writing and in conversation—and you consistently demonstrate professionalism, empathy, and sound clinical judgement.

Education & Licence Requirements
* Demonstrated recent experience as a Nurse Practitioner Candidate, Nurse Practitioner, or Advanced Practice Nurse.
* Current AHPRA registration.
* Nurse Practitioner endorsement, or current enrolment / willingness to enrol in a Master of Nursing (Nurse Practitioner) program.
* A valid driver's licence.
Desirable
* Previous Community Nursing experience.
* Experience working with clients with chronic and complex conditions in home or community based settings.
* Specialist knowledge in Chronic Heart Failure and Chronic Respiratory Disease.
