As part of our growth strategy, we are seeking a highly motivated and experienced Support Coordinator or Social Worker with a strong understanding of the NDIS framework and established connections within the disability sector, particularly with hospitals and healthcare providers.
The successful candidate will play a pivotal role in expanding our client base by connecting NDIS participants to our services and ensuring our SIL houses are fully occupied. Key Responsibilities
NDIS Participant Acquisition
* Proactively identify and engage NDIS participants who may benefit from our services.
* Build and maintain strong relationships with hospitals, healthcare providers, and other stakeholders to identify potential participants in need of SIL accommodations or other support services.
* Attend networking events, conferences, and community outreach programs to promote the company's services and establish a strong presence in the disability sector.
* Develop and implement strategies to attract new clients, including targeted marketing initiatives and partnerships with local organizations.
Support Coordination
* Work closely with NDIS participants to understand their unique needs, goals, and aspirations.
* Develop and implement individualized support plans in line with NDIS guidelines, ensuring that participants receive the services and resources they require.
* Coordinate and connect participants with appropriate services, including SIL accommodations, therapy, and community programs.
* Monitor participants' progress and adjust support plans as needed to ensure they are achieving their goals.
SIL House Occupancy Management
* Ensure all Supported Independent Living (SIL) houses are fully occupied with suitable participants.
* Conduct comprehensive assessments to determine the compatibility of participants with available SIL accommodations.
* Liaise with participants, their families, and support networks to facilitate smooth transitions into SIL housing.
* Work collaboratively with the operations team to ensure that SIL houses are well-maintained and meet the needs of participants.
* Conduct all participants' risk assessment before and after onboarding.
Compliance and Documentation
* Ensure all activities and processes comply with NDIS regulations and company policies.
* Maintain accurate and up-to-date records of participant interactions, support plans, and progress.
* Prepare detailed reports for the CEO and other stakeholders as required.
* Participate in audits and reviews to ensure compliance with NDIS standards and best practices.
Client Advocacy and Support
* Advocate for participants' rights and ensure they receive the highest quality of care and support.
* Provide ongoing support to participants and their families, addressing any concerns or challenges that arise.
* Act as a liaison between participants, their families, and service providers to ensure effective communication and collaboration.
* Promote the principles of choice, control, and inclusion in all aspects of service delivery.
Professional Development and Team Collaboration
* Stay up-to-date with changes in NDIS policies, procedures, and best practices.
* Participate in training and professional development opportunities to enhance skills and knowledge.
* Collaborate with colleagues to share insights, experiences, and strategies for improving service delivery.
* Contribute to the development of policies, procedures, and initiatives that support the growth and success of the organization.
Qualifications and Experience
* Essential:
* Degree in Social Work, Disability Studies, or a related field.
* Proven experience working with NDIS participants and a deep understanding of the NDIS framework.
* Strong connections with hospitals, healthcare providers, and other stakeholders in the disability sector.
* Excellent communication, interpersonal, and negotiation skills.
* Ability to work independently and manage multiple priorities effectively.
* Valid driver's license and willingness to travel as needed.
* Desirable:
* Experience in Supported Independent Living (SIL) accommodations.
* Knowledge of local community resources and services for people with disabilities.
* Experience in business development or client acquisition.
* Certification or training in NDIS Support Coordination or related areas.
