About the Role
As a health case manager in our team you will work with children, young people, their carers and caseworkers as part of a team providing holistic, trauma informed health assessment and planning. Our team works across the local health district to provide services, so you have opportunities to get out and about and develop a large network of supports for our clients. You will also provide health education to children and young people and their carers along with advocacy to help meet their health needs.
This role works in a supportive team alongside our Paediatrician, Nursing, Allied Health, Case Management and Administration staff. Orientation and training is provided with ongoing support to appropriate professional development.
Where You'll Be Working
This position is part of a dedicated multi-disciplinary team providing health and development assessment, liaison and planning for children and young people in out of home care. The team is passionate about improving health outcomes for children and young people.
About You
You must have a degree in Social Work, Nursing, Occupational Therapy, Physiotherapy, Psychology, Speech Pathology which provides eligibility for membership to the relevant professional association or full registration through AHPRA and a minimum of 3 years post-graduate experience relevant to this role with children and young people. You will also need to be able to drive in NSW and have a willingness to travel, sometimes long distances, for work purposes.
You will be experienced in providing child and family centred care with a holistic and trauma informed perspective. You will have great communication and interpersonal skills and enjoy working collaboratively with others as part of a team. Your clinical expertise will make a valued contribution to the team's ability to support and improve health outcomes for vulnerable children and young people.

Additional Information
An eligibility list will be created for future temporary part time vacancies.
To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.

Demonstrated experience working with Children/Young People within a multidisciplinary team environment, with a focus on child development, assessment (within scope of practice), and the creation/implementation of care plans.
Understanding and demonstrated experience of the principles of culturally competent healthcare when working with Children/Young People and Carers who identify as Aboriginal and/or Torres Strait Islander.
Demonstrated high level of communication skills, including experience with co-ordinating, networking and liaising with multiple key stakeholders and the effective use of available technology as an adjunct to communication.
